Admission process

Determining the need for admission

Examinations and tests are performed on an outpatient basis to determine whether admission to the hospital is necessary. If a decision is made that you need to be admitted, the hospital will notify you once the admission date has been set. On the day of admission, please go to the hospital reception desk at the time indicated in the notification.

Admission procedure

Please carry out the admission procedure at the Medical Affairs Office in the Secretary Division . It is to be carried out by the patient or a representative of the patient.
Please select a person whose identity is certain to serve as a guarantor, and a joint guarantor, then complete and sign (or endorse with your signature stamp) the Hospital Admission Application Form, Letter of Guarantee, and Medical Fee Payment Guarantee Form. In addition, please provide contact information that can be used during your hospitalization.

Documents needed for the admission procedure

  • Health insurance card, Eligibility Certificate for Ceiling-Amount Application
  • Consultation card (patient registration card)
  • Hospital Admission Application Form (Medical Fee Payment Guarantee Form)
  • Medical history sheet for admission, food allergy questionnaire
  • Signature stamp (may also been needed after admission) *Self-inking stamps cannot be used.
  • Discharge certificate (needed for patients discharged from a previous hospital within 3 months before admission)
  • The paperwork cannot be processed if you forget your health insurance card. Please note that you may be responsible for full payment until you present your insurance card.

  • Toiletries (e.g., shampoo, rinse, body soap)
  • Chopsticks, teacup
  • Tissues (Kleenex)
  • Bath mat (towel)
  • Indoor shoes (To prevent falls, bring sneakers, hallway slippers, nursing shoes, or the like.)
  • Rental items are not available at this hospital.

Hospital rooms

Four-bed rooms and special suites are available.
Please consult with the physician in charge if you wish to stay in a special suite. Patients who stay in a special suite are responsible for the full room fee. Please note that availability will determine whether a patient can use a special suite.
If a patient moves to another room, the fee for a special suite will apply if the patient moves to a special suite. This fee is not charged if the patient moves to a 4-bed room.

Billing

  • No deposit is required at the time of admission. Charges incurred during hospitalization are paid at the accounting desk on the 1st floor. Bills are calculated based on charges incurred through the end of each month and are sent in the middle of the following month.
  • In the evening of the day before discharge, the hospital will provide a statement indicating the approximate hospitalization charges. On the day of discharge, please pay the amount indicated at the accounting desk on the 1st floor. You can pay by cash or various credit cards.
  • Please present your Eligibility Certificate for Ceiling-Amount Application on the first day of that month. If you present it mid-month, it will apply to the following month.
  • The staff in charge will explain the billing process on an individual basis to patients receiving advanced medical care.

Visits

Anyone visiting a patient must report to the staff station.
The hospital cannot answer questions about a patient's hospital stay over the telephone. Please inquire with the patient's family members.

Preventing medical errors and infection

  • To prevent patients from being misidentified, physicians, nurses, and other medical staff confirm the patient's full name. Visitors are also asked for their own full name, and their cooperation is appreciated.
  • To prevent infection, patients and visitors are asked to use the quick-drying hand disinfectant located at the entrance to each room of the hospital when entering and leaving the room.

Facilities in the hospital, wards, and hospital rooms

  • A public telephone is located on the 1st floor.
  • Although basic daily necessities can be purchased at the shop, the selection is small. Therefore, patients are asked to prepare the items they will need before admission. The shop's hours of operation are 8:30 a.m. to 2:00 p.m. on weekdays. You can pay by cash or PayPay.
  • The washing machines and driers can be operated using a prepaid card.
  • You may purchase a prepaid card in the hospital ward. Please use the TV (free of charge in special suite A), phone (special suite B and C), washing machines, and driers with a prepaid card.
  • If you wish to use the shower room (9:00 a.m. to 6:30 p.m.), please ask a nurse.

Other important points

  • Smoking is not permitted in any area of the hospital.
  • The use of cell phones in the hospital can interfere with the functioning of medical equipment. Therefore, please use them only outside the hospital or in the designated phone corner on each floor.
  • Please note that the hospital generally does not relay telephone messages or faxes from outside the hospital.
  • A mailbox is located to the right of the public telephone on the 1st floor. Mail is picked up at 10:00 a.m. and 2:00 p.m. on weekdays.
  • When sending packages by parcel service or similar means, please include the name of the ward in the address.
  • Please do not arrive for admission by car and leave the car parked in the parking lot.
  • There is an ATM in the hospital. To prevent theft or loss during hospitalization, it is recommended that you bring only as much cash as necessary and use the ATM as needed.
  • Credit (Visa, Master, JCB, etc.) can be used at the hospital (except for some corporations).
  • You will be given a Hospitalization Guide during the admission procedure. Please use this as a reference.
